Collection, Use and Security of Personal Information

Philippine Association for Sport and Exercise Science acknowledges and respects every individual’s rights to privacy and confidentiality with regard to all private information. Also, PHASES gives importance to the collection of Personal Information that would facilitate an efficient product, service, and information delivery system to help enhance the practice of scientific application of the fields related to Human Movement Science. This policy encompasses all activities of PHASES, individuals visiting the PHASES website, signing up for PHASES mailings and periodicals, or commencing transactions with PHASES, and covers the gathering, utilization, storage, and disclosure of data and information purported in this Privacy Policy.

A. PHASES Collection of Personal Information

1. PHASES will only collect information furnished by the individual to PHASES. Individuals who have been referred to PHASES may also be subject to information gathering for e-mail purposes PHASES effort to disseminate information to greater populations. If a person donates to the PHASES, the PHASES may collect sensitive Personal Information such as the donor’s credit card number, card type, and expiration date and keep a record of the financial transaction.
Individual and Third Party Demographic information may also be collected for fund raising purposes and to notify persons of PHASES events, projects, educational opportunities and upcoming meetings.
The PHASES may collect Aggregate and Transactional Information and add to its database every time a person visits PHASES Web Site for research purposes.

2. Types of information: Personal Information is Demographic Information which identifies a specific individual with a minimal degree of effort. Demographic Informationincludes name, address, city and other similar information. Transactional Information is data collected on an individual based on the individuals' interactions with the PHASES, which may include sensitive information such as credit card information and Medical Information. Aggregate Information is information presented in summary or statistical form which does not contain data that would permit the identification of a specific individual without extraordinary effort.

B. PHASES Use of Personal Information

1. Personal Information furnished by individuals to PHASES may used for PHASES programs, research and fund raising.
2. The PHASES uses Transactional Information for research purposes for the development or implementation of its programs and services. It is, however, presumed that Transactional Information being highly proprietary will not be disclosed to third parties.
3. PHASES will disclose all information as required by law.
4. PHASES will discontinue the utilization of an individual’s personal information as soon as possible once requested.
5. PHASES reserves the right to retain Information in its archives and records to comply with law, resolve disputes, analyze problems, assist with any investigations, enforce PHASES’ User Agreement and other policies, and take other actions otherwise permitted or required by law.

C. Specific Requirements

1. Administrative

a. Personal information shall be kept in a secure environment and PHASES will take appropriate action in the event of unauthorized disclosure.
b. Personal Information access shall be limited to only those personnel with a legitimate business purpose.
c. PHASES owns all Personal Information provided to it by individuals and collected in accordance with this Policy.

2. Scientific Research

Researches funded by PHASES that involves human subjects shall be properly endorsed to appropriate institutions and ensure the strict compliance to research ethics.

3. Third Party Disclosure

Third parties requesting for personal information must first secure permission.
