*C – Sport Coaching; SC – Strength and Condtioning; SS – Sport Scientist
*failure to complete any of the requirements will delay the release of accreditation result

application fee is 300 (non-PHASES' members)/ 200 (PHASES' members) payable to a PHASES' representative on a specified seminar/seminar-workshop.

Certification renewal occurs after 2 years.
